MusiCares maintains the privacy and confidentiality of all clients’ medical records and personal information in compliance with local, state, and federal laws and regulations, including, but not limited to, the Health Insurance Portability and Accountability Act (including the federal regulations implementing such law), as amended by the Health Information Technology for Economic and Clinical Health Act, and as it may subsequently be amended.  MusiCares will not use or disclose clients’ medical records to any other third party, except as required by law. MusiCares takes all reasonable measures to protect the clients’ medical records from theft, loss, unauthorized destruction, and unauthorized access. 

Documentation & Immigration Status 

While MusiCares is only able to provide support to U.S.-based music professionals, please note that MusiCares does not require or request documentation related to immigration status. MusiCares also does not require you to provide a Social Security number as part of your request.


Independent Verification

MusiCares reserves the right to conduct an independent verification of information and documentation provided as part of this request. This verification may include, but is not limited to, review of public records, contact with other aid organizations with which MusiCares has an established relationship, and/or third-party entities or individuals named or referenced in this request. MusiCares may require additional contact information for third parties to complete this verification.


Fraud Prevention Notice

All applications are subject to review and verification. MusiCares reserves the right to delay, deny, discontinue, or recover assistance if fraudulent, false, or misleading information is suspected or identified. Where appropriate and in accordance with applicable laws and organizational policies, MusiCares may share relevant information with partner organizations, funding partners, or regulatory agencies to investigate or prevent fraud.

Why do we ask for this?  MusiCares protects charitable funds to ensure we can support eligible music professionals, and we request this information to verify eligibility.  Your information will not be used for purposes beyond identity verification. All photo IDs are stored in a secured and protected internal database. Only authorized staff have access to this database and are required to abide by HIPAA regulations to maintain highest levels of confidentiality.
